- If you are not prepared for your appointment, it will either be delayed or re-booked.
Ability to Understand English
- If you cannot speak English well, please bring someone with you who can translate English in your native language.
24 Hours Before Your Appointment
- Please Call or Email us to CONFIRM your appointment 24 hours before your appointment
On the Day of Your Appointment
- Do Not eat 6 hours before your appointment
- Do Not drink 2 hours before
- Arrive 30-45 minutes before your appointment time
- If you are LATE more than 30 Mins.
- Your appointment maybe cancelled.
- If you are LATE more than 30 Mins.
- Please call us when you arrive
- We will let you know when you can come upstairs and check in.
- Bring with you:
- Photo ID
- Ontario Health (if you have one)
- Socks or Slippers
- Nightgown or Long Shirt
- This is required for the Ultrasound in both MEDICAL and SURGICAL
- Surgical Mask (or a Cloth Mask)
Appointment Wait Times
- Due to the complexity of the appointment, appointment time may be 2-4 hours
Non OHIP Patients
- If you have Private Health Insurance
- Print all of your private insurance forms and bring them with you

- Please refer to the Block Fee Statement for the amount.

Transportation to/from the clinic
- Please DO NOT DRIVE to or from the clinic
- Please arrange to have someone drive you.
Parking
- Paid parking is available for $12 (CREDIT card only).
Childern and Support Partners
- No children are allowed in the clinic (arrange child care)
- Our in office waiting area is patients only, if someone comes with you, please ask them to wait in the downstairs lobby
Drugs and Medications
- Do Not Use any Recreational drugs 24 hours before appointment
- If you are taking any medication, please take them as usual with few sips of water
- Bring the list of medications you are taking.
- If you have ASTHMA, bring your puffers.
- If you are DIABETIC please bring your glucometer, strips and all medications
Feeling Unwell?
- If you are feeling unwell, cough, feverish, vomiting or diarrhea please call us, we may need to reschedule the appointment.
- Let us know if you have been out of the country within the last 14 days or have been in close contact with any one who has had COVID – 19 in the last 14 days.
